Best way to collect information via the internet is to make a spreadsheet with Google Documents (Now called Google Drive). I am an Apple guy myself but Apple doesn't yet have the capability to do what I need yet. So first you make a spreadsheet in Google Drive. Make sure you have everything you need there before you make the form. Then add a form at the top and there you have it. You can send it out to the people whom need to fill out information and easily collect their information without actually typing things yourself. :) Saves a lot of time and energy.
